PARTICIPANT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, INDEMNIFICATION AGREEMENT, MEDICAL AUTHORIZATION, AND ELECTRONIC SIGNATURE AGREEMENT
PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.
1. ASSUMPTION OF RISK: I understand that participating in athletic activities, specifically competitive basketball games, involves inherent risks including bodily injury, sprains, fractures, heat illness, permanent disability, and death. I voluntarily assume all such risks.
2. RELEASE OF LIABILITY: I release, waive, forever discharge, and covenant not to sue GMAA, its board, organizers, volunteers, facility hosts, and sponsors from any and all liability arising out of my participation.
3. MEDICAL AUTHORIZATION: In the event of injury, I authorize GMAA personnel to administer first aid and secure professional medical treatment or emergency transport.
4. MEDIA RELEASE: I grant GMAA permission to record my likeness, photographs, and video during tournament play for non-commercial promotional and community reporting.
5. ELECTRONIC SIGNATURE: By typing my full legal name and submitting this form, I affirm I am at least 18 years old and that my signature constitutes a legally binding agreement.
